  • What is the Online Self Service Form Preparation Program?

    • This online program can assist you in filling out certain forms used in selected court filings. This program does not give legal advice.

  • What types of forms are supported by the Self Service Form Preparation Program?

    • The Program allows you to complete the forms necessary to file:
      • Small Claim (excluding Auto Negligence and Return of Security Deposit)
      • Tenant Eviction (Residential Possession Only)
      • Dissolution of Marriage


  • How does the Self Service Program work?

    • You will be asked a series of questions related to the case you are filing. Answer these questions with as detail as possible. The information you provide will be used to complete the appropriate forms. This program does not check spelling or grammar. The information on the forms will appear just as you entered it.

  • How do I get started?

    • Once you have read and accepted the Self Service Program Disclaimer and User Agreement a brief instruction page is displayed. On the following page, you will select the type of case you wish to file. General information regarding the selected case type is displayed, including the information that is required to complete the questionnaire. This is followed by the Fee Schedule, including the Service Charge for using the Self Service Program and the Filing and Process Service fees necessary to initiate your case. On the next page, you will create an account name and password.

  • Do I have to pay to set up an account?

    • No. There are no charges until you have completed all the questions and are ready to print the completed forms.

  • Do I have to complete all the questions at the same time?

    • No. Once you establish an account, the forms will be saved and can be accessed for seven (7) days. If you have not completed the questions, the program will return you to the uncompleted questionnaire so you may continue at the next login.

  • How long is my information kept in the system?

    • If there is no account activity for over 7 days, the information is deleted. If the questionnaire has not been completed, the process must be restarted from the beginning.

  • How do I change the information/answers I gave on my forms after I’ve printed them?

    • Using your user name and password, you will be able to view, edit and re-print your documents for 7 days.

  • Why can’t I make changes to the forms directly when I’ve completed the process and are reviewing them?

    • You may not edit the forms directly. To change your information, click the appropriate category in the left Navigation Bar, edit your work, and return to the “Forms” page. The changes you made will now be reflected on each individual form.

  • Does my user ID and password remain in the system even after the 7 day period?

    • Yes. You can use the same user ID and Password after 7 days, but you will be entering new information and paying again for the service.

  • Can I get these forms somewhere else?

    • If you would rather not use the Self Service application, you may visit the Clerk’s Website www.browardclerk.org and click on the Report and Form Download tab to access the various .PDF form packages. Or, you may visit one of our offices and purchase individual forms for $.15 per page.

  • What is the difference between the download and self-service forms?

    • The downloadable Clerk Forms Packages are blank forms in Portable Document Format (PDF) that, once printed, need to be filled out by the user. The Self Service forms are completed using your responses to the questions asked in the online process. When these forms are printed, they are ready to be signed and filed.

  • Can these forms be filed electronically?

    • No, the Broward Clerk’s Office does not support electronic filing at this time.

  • Where are the completed forms filed?

    • A Petition for Dissolution of Marriage is filed in the Circuit Civil/Family Division at:
